They will need to make informed decisions on your behalf.

  • Identify Your Decision-Makers: Think about who you trust to make decisions on your behalf. This could be a family member or a close friend. 

  • Document Your Wishes: Write down your preferences for your end-of-life care. This can include the types of treatments you do or do not want. In addition, fill out your state’s advance directive form

  • Share the Information: Make sure your decision-makers, family members, and healthcare providers have copies of your documents.
